The mission of JamCAN Home Care is to provide quality affordable home care and companion services to the elderly or those in need of physical assistance as a result of surgery, chronic disease or after giving birth. JamCAN provides tailored services depending on the needs of the clients including short and long term arrangements, daytime and after-hours care. Care may be provided by different types of caregivers (companion, practical nurses, and/or physician) depending on the needs of the clients. JamCAN is dedicated to providing care for clients as we would our own families. Through the use of technology the JamCAN care provider is focused on the overall wellbeing of each client, closely monitoring needs and reporting conditions to loved ones, physicians and supervising nurse practitioners to ensure the most appropriate care is provided at all times. JamCAN is dedicated to providing clients, their families and primary care physicians with up-to-date information on the status of care and physical condition, which will ultimately ensure the best care for loved ones both at home and with clinical primary caregivers. In the case of hospitalization due to illness or surgery, JamCAN will ensure that patient information is communicated in a timely fashion and will work with clients upon their return home to decrease the likelihood of readmission.
